Payment Process Procedures:

Credit Card Payment Procedure:

Your credit card is validated at the time your order is placed and an approval confirmation email will be sent to you. For credit cards that do not authorize, a notification will be provided via email to submit an alternate form of payment. Your order will not be processed without an approved credit card payment.

Purchase Order Payment Procedure:

Verify that your Purchase Order is valid and contains the following information:

  • Purhase Order Number
  • Customers Company Name
  • Customers Billing Address
  • Customers Shipping Address
  • Total Dollar Amount
  • Authorized Signature

Fax your enrollment form and purchase order to 978-461-3760 within 1 business day of placing your order or your order will be cancelled and you will need to reenroll. Faxes with missing information will not be processed.

Check Payment Procedure:

Fax within 1 business day of placing your order your enrollment form and a photo copy of your check to 978-461-3760. Orders not received within one business day will be canceled. Customers should mail their check and order form to:
